Every taxpayer in Lithuania can personally decide where a portion of their income tax goes.
It costs nothing and takes only a few minutes — everything is done through the official e-government portal.

That 1.2% can turn into real help for those who truly need it.
You decide where it goes.

By allocating 1.2% to Action4.life, you are simultaneously supporting:


Ukrainian defenders on the front line;

Civilians through cultural and educational initiatives;

The development of sports, health, and creativity among war-affected Ukrainians.

This way, you support three causes — with one simple form.
Less than five minutes for you — real help for them.

How to make the transfer

1. Go to the website: deklaravimas.vmi.lt
Click on «Prisijungti prie EDS», to log into the system

2. Authorize yourself
Use internet banking, a mobile signature, or another convenient method.
 
3. Enter the EDS system
On the main page, select the section «VMI sistemos → EDS (Elektroninio deklaravimo sistema)»

4. Proceed to filling out the form
Select the menu «Deklaravimas → Pildyti formą»

5. Find form FR0512 v.5
The form is titled «Prašymas pervesti pajamų mokesčio dalį paramos gavėjams ir (arba) politinėms organizacijoms»

⚠️ Important: If you had income outside of salaried employment (e.g., from rent or self-employment), first submit the GPM311 declaration, and only then fill out the FR0512 form.

Fill out the form:

  • Recipient: VŠĮ Action4.life

  • Organization code: 306070930

  • Percentage: 1.2%

Sign and submit the form by May 1, 2025.
